  • Impact of statin therapy on coronary plaque composition: a systematic review and meta-analysis of virtual histology intravascular ultrasound studies

    BackgroundVirtual histology intravascular ultrasound (VH-IVUS) imaging is an innovative tool for the morphological evaluation of coronary atherosclerosis. Evidence for the effects of statin therapy on VH-IVUS parameters have been inconclusive. Consequently, we performed a systematic review and meta-analysis to investigate the impact of statin therapy on plaque volume and its composition using VH-IVUS.MethodsThe search included PubMed, Cochrane Library, Scopus and Embase (through 30 November 2014) to identify prospective studies investigating the effects of statin therapy on plaque volume and its composition using VH-IVUS.ResultsWe identified nine studies with 16 statin treatment arms and 830 participants. There was a significant effect of statin therapy in reducing plaque volume (standardized mean difference (SMD): −0.137, 95 % confidence interval (CI): −0.255, −0.019; P = 0.023), external elastic membrane volume (SMD: −0.097, 95 % CI: −0.183, −0.011; P = 0.027) but not lumen volume (SMD: −0.025, 95 % CI: −0.110, +0.061; P = 0.574). There was a significant reduction in fibrous plaque volume (SMD: −0.129, 95 % CI: −0.255, −0.003; P = 0.045) and an increase of dense calcium volume (SMD: +0.229, 95 % CI: +0.008, +0.450; P = 0.043), while changes in fibro-fatty (SMD: −0.247, 95 % CI: −0.592, +0.098; P = 0.16) and necrotic core (SMD: +0.011, 95 % CI: −0.144, +0.165; P = 0.892) tissue volumes were not statistically significant.ConclusionsThis meta-analysis indicates a significant effect of statin therapy on plaque and external elastic membrane volumes and fibrous and dense calcium volumes. There was no effect on lumen volume, fibro-fatty and necrotic tissue volumes.

  • Impact of L-carnitine on plasma lipoprotein(a) concentrations: A systematic review and meta-analysis of randomized controlled trials

    We aimed to assess the impact of L-carnitine on plasma Lp(a) concentrations through systematic review and meta-analysis of available RCTs. The literature search included selected databases up to 31st January 2015. Meta-analysis was performed using fixed-effects or random-effect model according to I2 statistic. Effect sizes were expressed as weighted mean difference (WMD) and 95% confidence interval (CI). The meta-analysis showed a significant reduction of Lp(a) levels following L-carnitine supplementation (WMD: −8.82 mg/dL, 95% CI: −10.09, −7.55, p < 0.001). When the studies were categorized according to the route of administration, a significant reduction in plasma Lp(a) concentration was observed with oral (WMD: −9.00 mg/dL, 95% CI: −10.29, −7.72, p < 0.001) but not intravenous L-carnitine (WMD: −2.91 mg/dL, 95% CI: −10.22, 4.41, p = 0.436). The results of the meta-regression analysis showed that the pooled estimate is independent of L-carnitine dose (slope: −0.30; 95% CI: −4.19, 3.59; p = 0.878) and duration of therapy (slope: 0.18; 95% CI: −0.22, 0.59; p = 0.374). In conclusion, the meta-analysis suggests a significant Lp(a) lowering by oral L-carnitine supplementation. Taking into account the limited number of available Lp(a)-targeted drugs, L-carnitine might be an effective alternative to effectively reduce Lp(a). Prospective outcome trials will be required to fully elucidate the clinical value and safety of oral L-carnitine supplementation.

    The adoption of innovative wearable technologies is potentially increasing as a new trend. Jumping into the augmented reality (AR) and Metaverse, Facebook (now known as Meta) launched smart glasses partnering with Ray-Ban sunglasses brand’s parent company EssilorLuxottica. Ray-Ban stories has several technical features for entertainment and socializing; more importantly, these features can be adopted in the future for more advanced wearable. However, these smart glasses also came with many ethical and privacy concerns along with their potential benefits. Furthermore, the unbridled deployment of these smart glasses brought several challenging questions for public social interaction when we will have more such devices in our lives. This short article has discussed the Ray-Ban stories’ ethical and privacy issues for social interaction and public places.

    Background Wearable smart glasses offer new opportunities to enhance limb preservation surgery by enabling hands-free photo/video capture, real-time communication, and improved surgical education. While earlier devices such as Google Glass demonstrated feasibility, they were hindered by limited battery life and cumbersome ergonomics. The new Ray-Ban Meta smart glasses may address these limitations by providing high-definition recording in a lightweight, ergonomic frame. Recent consumer-grade smart glasses may address these limitations by providing high-definition recording in lightweight, ergonomic frames. Methods From November 2023 to April 2025, 2 limb preservation surgeons used Ray-Ban Meta consumer-grade smart glasses during foot and ankle procedures. Five domains were evaluated: overall use and feasibility, image and video utility, workflow and communication impact, team and trainee feedback, and technical performance. Patients were enrolled based on procedural scheduling and availability for device setup, with written consent obtained preoperatively. Results The smart glasses were easily integrated into operative workflows, enabling hands-free documentation without disrupting sterility. Trainees found first-person footage more engaging than conventional video formats. Real-time streaming supported remote consultation, and occasional live transmission facilitated tele-mentoring. Although battery capacity decreased with frequent use, no major technical failures occurred over the study period. Conclusion This is the first comprehensive report on using Ray-Ban Meta consumer-grade smart glasses in limb preservation surgery. Early findings support their use as a low-profile, hands-free solution for intraoperative documentation, education, and communication. Further optimization—especially regarding battery endurance and data integration—will enhance the role of wearable devices in surgical care.
